Selbsteinschätzung des Arztes als ein Prädiktor für die Beziehungszufriedenheit stationärer Patienten

Zusammenfassung
Zielsetzung: Auf dem Hintergrund der Bedeutung der Arzt-Patienten-Beziehung für den Behandlungsverlauf, aber auch für das Ausmaß an Beschwerden gegen Ärzte und Krankenhäuser, wird überprüft, ob die Selbstbeurteilung von Ärzten hinsichtlich ihrer Kommunikation mit Patienten eine Vorhersage der Qualität der Arzt-Patient-Beziehung aus der Sicht von Patienten ermöglicht. Methodik: In einer Fragebogenstudie am Allgemeinen Krankenhaus der Stadt Linz (Teilnehmer: 84 Ärzte und 1804 Patienten von 14 Abteilungen) schätzten Patienten die Qualität ihrer Beziehung zu den sie behandelnden Ärzten ein. Ärzte wiederum bewerteten sich selbst hinsichtlich ihrer Kommunikation mit Patienten. Mittels hierarchischer Regressionsanalyse wurde getestet, in welchem Ausmaß sich die Beziehungsqualität (Patientenurteil) aus dem Arztverhalten (Selbstbeschreibung der Ärzte) vorhersagen lässt. Ergebnis: 29 % der Varianz der Beziehungsqualität konnten durch die ärztliche Selbstcharakterisierung hinsichtlich kommunikativer Verhaltensweisen vorhergesagt werden. Schlussfolgerungen: Aufgrund dieses Ausmaßes an Varianzaufklärung und da schon in früheren Arbeiten gezeigt werden konnte, dass die ärztliche Kommunikation positiv beeinflussbar ist, besteht eine realistische Chance, durch zusätzliche Trainings Verbesserungen der Arzt-Patient-Beziehung zu erzielen.
Abstract
Aim: Under consideration of the importance of the doctor-patient relationship for the treatment-process in general, as well as for legal actions taken against doctors or hospitals, it is tested, whether physicians’ self-descriptions of their communicative skills are predictors for the quality of the doctor-patient relationship as perceived by patients. Method: In a comprehensive questionnaire-study at the General Hospital of Linz, Austria (AKH-Linz; Participants: 84 physicians and 1804 patients from 14 departments) patients rated the quality of their relationship with their doctors, who, on the other side, rated themselves with respect to their communicative skills in medical encounters. By use of hierarchical regression analysis it was tested whether the physicians’ self-descriptions are powerful predictors of the quality of the doctor-patient-relationship – as perceived by the patients. Result: The physicians’ ratings of their communicative skills explained 29 percent variance of the quality of the doctor-patient relationship. Conclusion: Due to the amount of explained variance, and because prior studies reported positive outcomes of communication-skills training in physicians, it may be assumed that additional training may influence the quality of the doctor-patient relationship in a positive way.
